What We Do

Diabetes Education and Self-Empowerment

Our program seeks to enhance the health of individuals with or at risk of Type 2 Diabetes through a comprehensive approach that includes evidence-based education, self-management support, and community interventions. By promoting lifestyle like diet and exercise, aim to help individuals achieve and maintain healthy blood sugar levels and reduce long-term risks.

Cardiovascular Disease Care Coordination

Our program tackles high blood pressure, a leading cause of heart disease, by providing education, self-monitoring tools, and care coordination to at-risk adults. This initiative aims to significantly reduce the number of people with uncontrolled blood pressure, lowering their risk of cardiovascular disease.

Education and Outreach

Our mission is to improve health equity in the Alabama Delta. We achieve this by empowering communities through education and outreach programs. We connect residents with essential resources, promote healthy lifestyles, and work to eliminate barriers to accessing quality healthcare. Our goal is to create a healthier future for all members of the Delta region.
